Why would the answer not be 3?

If Fall O is folder 2, then Spring O is folder 1 which puts Fall math in folder 4. Folder three and folder four cannot have the same subjects so folder three must have nutrition. That would mean we know folders 1,2,3, and 4 being O, O, N, M, _, _. Did I read the premises wrong? Can you explain why the answer is 2?

I find this is be a bit ambiguous, and your analysis is spot on. I think that when the question refers to course offerings, it means Ns or Of, as in both the semester and the subject. While we know the subject for N, that n could be in either semester, so we only know about the second o and m for sure. That said, this really is ambiguously worded, but when in doubt, go back to the setup for how they used the term course offering. Since the setup mentions the six course offerings, I would read this as meaning how many of the six we can place in full.

I am not quite understanding what the question is asking. Can you please clarify?

The question is asking us how many of the folders we can place for sure once we have placed the Fo in 2. Just a note on my earlier reply, I think the initial question was confused about how many total elements there were (three or six). For this question however, we are explicitly asked how many of the remaining five we can place, so not including the Fo in two.
