I don't understand the even a doubling of the rate of disorder Y. Does it mean that even if the disorder Y has 0.04 of the population, the two studies still need more then 1100 people? Thank you!

That's correct. This part of the argument refers to the fact that the disorder is so rare - only occurs in .02 percent of the population that even if it was double that - 0.04, 1100 people still would not be enough to detect it.
